New ACFI features shine in Clinical and Care Management

At Telstra Health – Aged, Disability & Community Care (ADCC), we work hard to produce residential aged care software that creates efficiencies, maximises funding opportunities, reduces compliance risks and enhances the quality of care for residents.

That’s why we developed the latest version of Clinical and Care Management software, which includes significant enhancements to the Aged Care Funding Instrument (ACFI), Care Plan and Assessment Form areas. This release is our biggest to date, with a large focus on User Experience (UX) to ensure our software is easy to use and requires minimal training. These enhancements are in addition to Clinical and Care Management’s core functionality, including ACFI management, clinical forms, medication management, occupancy management, and reporting. With the resources of Telstra Health, our clients will continue to benefit from considerable software enhancements into the future.

ACFI management

Clinical and Care Management now includes an enhanced ACFI module. The redesigned ACFI pack contains an intuitive user interface which was developed in line with client feedback. To help ensure reappraisal due dates are never missed, new monitoring functionality ensures submission dates are always visible.

With the new ACFI Dashboard, users are notified of the number of residents with a potential increase in their funding. Enhanced ACFI alerts functionality now notifies users of any completed clinical documentation that may be used as evidence for a potential increase to a resident’s funding.

Identifying residents with potential uplifts has been made easier with the help of evidence based recommendations. With Clinical and Care Management’s Quick Calculator, users can promptly estimate a resident’s potential appraisal uplift and identify relevant category increases. In addition, users can now save the Quick Calculation for a resident to auto-populate an ACFI Appraisal Pack, speeding up the ACFI process.

Enhanced reporting functionality enables users to identify any variance in resident funding or congruence errors. New printing options allows users to easily and quickly print each piece of evidence attached in the resident’s ACFI pack from one page.

Care Plans

The latest release of Clinical and Care Management has been designed to provide a high level of accuracy and consistency of information within Care Plans. This includes an enhancement to the dynamic flow of information from Assessment Forms to Care Plans, ensuring that data is always consistent.

With the rise of person-centric care, Clinical and Care Management now includes superior care domain and sub domain management with drag and drop features. This functionality enables users to highlight and group the aspects of a Care Plan that are the most important for each individual resident.

Assessment Forms

The latest version of Clinical and Care Management contains considerable enhancements to the user experience within Assessment Forms. Users can now easily identify which questions are mapped to the Care Plan, and navigate to other relevant clinical documentation while completing a form. In addition, Clinical and Care Management now contains a process flow which automatically generates a Progress Note when an Assessment Form is saved as unfinished.

Resident Manager

Residential aged care providers can now manage their resident administration, leave process, and Medicare funding with the new Resident Management software. Resident Manager streamlines the management of Medicare B2B events, including admissions, departures, leave and ACFI submissions.

An integration between Clinical and Care Management and Resident Manager enables prospect information in Resident Manager to flow through to Clinical and Care Management prior to or upon admitting a resident. This real-time integration has been designed to reduce the likelihood of Medicare rejections, minimise the amount of data entry required, and ensure funding is up to date.

By using Clinical and Care Management and Resident Manager software, residential aged care providers have the tools they need to help streamline their day to day operations – from clinical care delivery through to funding optimisation and reporting.
